TERMS AND CONDITIONS June 2011

Referral method/contract
Referral can be verbally for individual assessments for private cases where the individual is paying directly.  For any work undertaken where a third party is paying, ie solicitor, insurance company or local authority, written instruction in the form of a short letter is required.  The party responsible for payment must sign this agreement/instruction.

The contract will be direct with Denise Winks and she will not involve other therapists or case managers without prior agreement with the instructing party.

Fees and travel rates

Professional time to include assessments and report writing	£78 per hour
Administrative work and telephone calls	£45 per hour
Travel	£35 per hour
Mileage is charged at 55 pence per mile

Time spent is charged to the nearest quarter hour.

Invoice and Payment terms
Work will be invoiced at completion of the assessment and intervention or monthly if intervention is ongoing.

Terms are strictly 30 days from the date of the invoice. 

VAT will not be added to invoices where VAT exemption applies. (This will be for the majority of work with the exception of reports for the Court). VAT will otherwise be charged at 20%.
